What Is a Choi Pen and Does It Make a Difference in Hair Transplant?

The Choi Implanter Pen is a medical instrument used to place hair grafts into the scalp during a hair transplant. Instead of first creating all recipient channels and then inserting the grafts with forceps, the Choi Pen allows the graft to be loaded into a hollow needle and implanted directly into the recipient area.

The Choi Pen is commonly associated with Direct Hair Implantation (DHI). However, DHI is not a separate hair-harvesting technique. In most DHI procedures, the hair follicles are still removed individually from the donor area. The main difference is how those grafts are implanted afterward.

What Are the Potential Advantages of Using a Choi Pen?

One practical advantage is reduced direct handling of the follicular graft during placement.

Angle control is the second advantage, and it is less about precision in the abstract than about reproducing something specific: the way real hair does not grow in one direction. A pen lets the surgeon set entry angle graft-by-graft, which is useful anywhere hair direction changes sharply over a small area, the crown whorl being the obvious example, but also the transition zone where frontal hair angle flattens into mid-scalp hair angle.

Another potential advantage occurs when transplanting between existing hairs. Because implantation can be performed in a controlled fashion, an implanter may be useful in selected patients with residual native hair rather than a completely bald recipient area.

Does a Choi Pen Give Better Control of Hair Direction?

It can give the direct control during implantation, but the result still depends on how that control is used. Direction is particularly important in the:

  • Frontal hairline: Hairs should emerge at appropriately acute angles and follow a natural, irregular distribution.
  • Temporal regions: Hair angulation generally becomes even more delicate, making incorrect placement conspicuous.
  • Mid-scalp: Direction should transition smoothly rather than creating abrupt changes.
  • Vertex: Grafts need to integrate into the patient's natural clockwise or counterclockwise whorl.

Recipient-site angle and direction should follow natural hair architecture. In the vertex specifically, grafts need to reproduce the whorl rather than being implanted in uniform rows. The pen is therefore capable of precision, but design creates naturalness.

What Matters More Than the Choi Pen?

Patients often ask which implanter pen a clinic uses. It is the wrong first question. A Choi pen, or any implanter, places grafts.

No device on the market can tell a surgeon whether a patient is a suitable transplant candidate, where the hairline should sit relative to facial proportions, how much donor hair can be harvested without thinning the back and sides, or how much future hair loss is likely over the next decade. It cannot design a crown whorl, or judge how a given patient's follicles can be used before donor density suffers.
